Indiana State Board of Dentistry

1] Create an application account: mylicense.in.gov/eGov/index.html —> “Personal Licensing” account *Hold onto Log-in Credentials!!

  • New Applicants —> “Examination” Process

  • Dentists with active license from other states —> “Reciprocity” option

2] Complete Application: Gather & Submit

  • Yes / No Questions section

  • Application Fee: $250 nonrefundable fee (Credit / Debit)

  • Name Change Documentations (if applicable)

    • Marriage / Divorce decree

    • Legal Name Changes

  • Official Dental School Transcripts: (IUSD Office of Academic Affairs)

  • Integrated National Board Exam scores: ADA

  • Proof of Clinical Exam Completion

  • Certification of Completion: IUSD Office of AA after you graduate

  • BLS (Basic Life Support)/ACLS (Advanced Cardiac Life Support) certification

  • Citizen or “Qualified Alien” Question

3] Criminal Background Check

  • *DO NOT start until you have submitted & received confirmation

  • Online instructions for CBC

  • Fingerprints must be submitted on/after the date of email notice*

4] Email-Invited Jurisprudence Exam for the State of Indiana

  • You MUST Pass this exam (75% criteria; no exemptions)

      • IC 25-13: Dental Hygiene Law

      • IC 25-14: Dental Law

      • Title 828: IAC Dental and Dental Hygiene Rules

      • Title 410 IAC 1-3 & 1-4: Infectious Waste & Universal Precautions

5] Notification of Licensure

  • You can download your license and order wall certificates immediately upon passing the jurisprudence exam.

When and Where can I take this exam?

Each state is different. In Indiana, you can begin after graduation - NOT BEFORE - and you have 1 year from when you start to complete it. Once the board emails you , you have 14 days to complete your jurisprudence exam. The exam can be taken at your home since it is emailed.

Does passing CDCA / other regional board allow you to work without taking the jurisprudence exam?
